Before Your Appointment

To help ensure a smooth intake process:

• All intake paperwork must be completed prior to your appointment.

• A valid credit or debit card must be kept securely on file.

• Insurance information must be uploaded before your appointment if you plan to use insurance benefits.

• Appointments may need to be rescheduled if required paperwork is incomplete.
   

Cancellation Policy

• Appointments canceled with less than 24 hours' notice may be subject to a late cancellation fee.

• Missed appointments (no-shows) may be subject to a no-show fee.
