1)New Clients: Thank you for requesting an appointment with us! Please know that all appointment requests must be reviewed and approved before they’re confirmed. Submitting a request does not automatically guarantee your appointment time. Once your request is reviewed, you’ll receive a confirmation email. A CREDIT CARD ON FILE WILL BE REQUIRED TO CONFIRM APPOINTMENTS AND CHARGED $60 IF APPOINTMENTS ARE CANCELLED LESS THAN 24HOURS OR NO SHOW.
2)Please complete all intake forms at least 24 hours before your appointment. Incomplete forms may result in cancellation or rescheduling.
3)Please ensure your insurance information is up to date. Clients are responsible for any fees not covered by insurance, including deductibles and copays. Appt are automatically cancelled if insurance not active. Questions? Contact our office before booking.
4)We do not provide long-term benzodiazepine or stimulant prescriptions without assessment and documentation of medical necessity.
