Kelli Cremeans, APRN, currently provides telehealth services to patients located in Kentucky, Colorado, and Virginia only. You must be physically located in one of these states at the time of your appointment.

Please ensure that all required intake forms and paperwork are completed in your patient portal prior to your visit so your provider can review your information in advance and make the most of your appointment time. Check your junk mail if you do not see your intake paperwork after booking your visit.

Evening appointments are available upon request — please contact us directly if you need a time outside standard business hours.

Phentermine Initial Consult Video Visit (KY & CO PATIENTS ONLY) $ 74.99

Phentermine is a prescription medication used to support safe, medically supervised weight loss. Because it is a controlled substance, a live video visit is required for all new prescriptions.

During your initial visit, your provider will review your health history, current medications, and weight loss goals to determine if phentermine is an appropriate option for you.

State and federal laws require recent lab work, blood pressure, and heart rate readings to be on file before treatment can begin.

If approved, your provider will develop a personalized plan to help you reach your goals safely and effectively.

Follow up-Phentermine Video Visit (KY & CO PATIENTS ONLY) $ 74.99

This visit is for established patients currently prescribed phentermine who need a follow-up evaluation or prescription refill.

During your video visit, your provider will review your progress, discuss any side effects or concerns, and adjust your treatment plan as needed to ensure safe and effective results.

Initial Weight Management Visit $ 74.99

Take the first step toward lasting, healthy weight loss with a personalized treatment plan built around your goals and medical needs. During your initial visit, your provider will review your health history, lifestyle, and lab results (if available) to determine the most effective options for you — including GLP-1 medications such as semaglutide or tirzepatide, or other evidence-based therapies.

Weight Loss Refill Request-THIS IS NOT A VIDEO VISIT. $ 74.99

Need a refill on your prescribed weight loss medication? This option is for established patients who are continuing their current treatment plan and do not require a live video appointment.
After submitting your refill request, your provider will review your chart, current dosage, and progress to ensure it’s safe to continue your medication. If additional information or an updated visit is needed, you’ll be contacted before your refill is processed.

Please note: This is an asynchronous service, not a live video visit. You must be an active patient with recent follow-up documentation to qualify.

Asynchronous Visit- A visit on YOUR time! THIS IS NOT A VIDEO VISIT $ 74.99

Get expert medical care on your schedule — no live appointment required. Our Asynchronous Visit option allows you to securely share your symptoms, health history, and goals through an online questionnaire.

A licensed provider will carefully review your information and respond with a personalized treatment plan, prescriptions (if appropriate), and follow-up recommendations.

During your virtual appointment, your provider will review your progress, address any concerns, and update your treatment plan as needed — ensuring your care remains personalized and effective.

For those interested in testosterone replacement therapy, use this link. The initial visit will include extensive labwork, in depth consultation of symptoms and concerns, testosterone medication as well as other medications if needed for treatment.

Please note: Testosterone replacement is currently only available in Kentucky and Colorado.
