Please note: 
-New patient appointment requests are reviewed before confirmation.
-A valid government-issued photo ID is required for all new patient visits.
-A valid card on file is required when requesting an appointment.
-Your card will not be charged when you submit the request. Once your appointment is approved and confirmed, a $25 booking deposit will be charged and applied toward your visit
-Please provide at least 24 hours' notice for cancellations or rescheduling. Late cancellations or no-shows may result in forfeiture of the $25 deposit.
-Laboratory testing may be required before your appointment, depending on the service requested.
-The remaining balance is due at the time of service.
