Booking this appointment will begin membership status and bill monthly for $50 which provides access to unlimited telehealth and office visits per month, point of care testing, and some pharmacy coverage.

Members will be put on recurrent billing automatically and must read and review the cancellation policy when choosing to terminate membership for any reason.

Members who cancel their membership before 3 full months of ?membership payment will not be able to re-enroll for a period of 3 months after cancellation.

MedWell and the practitioner has the right to refuse care based on patient health issues that require more extensive and specialized care that the practice cannot accommodate.

Patients may book this appointment for the following reasons: respiratory infections, ear infections, sore throats, sinus problems, minor injuries (sprains, strains, simple wounds), skin issues like rashes, eczema, or infections, urinary tract infections, GI issues like nausea, vomiting, reflux, headaches or migraines, allergies and seasonal flare-ups.

Patients may book this appointment for the following reasons: Ongoing management of hypertension, asthma care and inhaler management, COPD follow-up and symptom monitoring, diabetes management and medication adjustments, hypothyroidism or hyperthyroidism follow-up, arthritis and chronic pain management, mental health support for anxiety or depression, insomnia evaluation and sleep hygiene guidance.

All applicable records pertaining to their claimed condition should be provided at the visit, including: physical notes, surgical notes, medication lists, copies of diagnostics (MRI, Xray, Sleep Study, PFTs, etc), and a legible copy of your DD-214.
