You can fill out the intake forms online, To access our secure and HIPAA compliant patient portal (from our partner OptiMantra), please click on: https://www.optimantra.com/optimus/om/patient/login
Please click on the 'Set Password' button to set your password. A link will be sent to your email and from that link you will be able to create a password and then sign on to the Portal. You will also find the forms there.
Please fill the Demographic Details, Past Clinical History, Consent /tx financial hippa and on the left menu find medications and supplements.

After the forms are completed we can set up an appointment  and will verify your insurance. We do like to have a brief conversation before the appointment.

This is an initial intake and acupuncture. A treatment plan will be determined at this visit.

This is an initial visit for an herbal consultation. Please select this if you have not been seen before, or if it has been greater than three years since your last visit. Acupuncture is not included in this visit.
