Evaluation Requirement: Once completed, this evaluation is valid for 12 months. 

• All patients who have not been seen within the past year by ReNu Hormone and Wellness are required to complete a wellness evaluation prior to receiving IV therapy, vitamin injections, or other treatments. This assessment is performed to determine medical appropriateness and ensure patient safety
• This does not include the medication, IV therapy or Vitamin injections.
