The first time you sechedule online, you will be asked to leave a card on file and prompted to pay. After confirming your appointment, you will be sent an email with directions to sign into the portal to complete your online forms.

*Douros Healthy Weight Institute will not be billing insurance directly. However, a superbill/receipt will be created for each patient and accessible in the portal. Patients will be able to submit their bills for reimbursement and/or apply this money towards their deductibles.
The Legacy Patient Package is reserved only for current patients that are following Dr. Douros to the Douros Healthy Weight Institute. The program is 200 dollars a month for 6 months (with a 6 month contract) during the active weight loss stage. Then, the visits will be 200 dollars a month thereafter until the patient reaches their goal weight. Once at goal weight, the patient will switch to the Maintenance Package which is at 200 dollars a month every other month for one year. Once the one-year maintenance package is complete, the patient becomes a lifetime member and will be able to see me as needed for 200 dollars a visit at an appropriate interval depending on their needs to optimize the maintanance of their weight loss. If the patient is on medication for weight maintenance, there will be a required visit every 6 months.
Although Douros Healthy Weight Institute does not directly bill insurance, Dr. Douros will provide you a Superbill after each visit that will link your monthly payment to the visit. The Superbill will contain all the diagnostic codes and service codes for you to turn into your insurance. Depending on your plan, the insurance may apply the visit towards your deductible, reimburse you, or you may use your HSA savings accounts to pay for the visits.
The Legacy Maintenance Package is reserved only for patients that are following Dr. Douros to the Douros Healthy Weight Institute. It is for patients that have reached their goal weight and are in the critical maintenance phase of their weight loss. This plan is 200 dollars every other month for one year with visits every 8 weeks and the support services needed to successfully maintain their weight loss. Once the maintenance phase is complete the patients move to the Lifelong Membership phase. This means they can follow up with me as needed for 200 dollars a visit for support in their weight maintenance, with a required visit every 6 months if they are on maintenance medication.
