All new patient evaluations must be in person. 

These follow up appointments can be in person or virtual through HIPAA compliant videoconference according to patient preference. All 45 min follow up appointments will be presumed to be in person. If you prefer virtual, please notify your provider through the patient portal as soon as possible.
