This appointment requires a non-refundable/not transferrable deposit of $150 which covers your initial evaluation and review of medical history. The $150 deposit can be applied to any treatment plan or medication. *Note that if you have been absent from the program for 6 months or more, you will be required to pay a $150 initial consultation fee to begin your program again.
Labs Required: Labs must be completed and resulted prior to your initial consultation so plan to get your labs drawn as soon as possible. A lab order will be submitted once your appointment is booked.
