Whether this is your first visit or recurrent visit with us, please plan to arrive
15 minutes early to allow time to complete new patient paperwork. A member of our staff may also reach out to you prior to your appointment to gather additional health information and ensure we are fully prepared to care for you.
What to Bring
Please bring a
valid photo ID and your
insurance card(s) to every appointment. We also encourage you to bring a current list of your medications, including dosages, as well as any relevant medical records or referral documentation that may be needed for your visit.
Insurance & Payment
We accept most major insurance plans. Prior to scheduling your appointment, please verify that our practice is
in-network with your insurance provider to avoid unexpected out-of-pocket costs. Co-pays and any outstanding balances are due at the time of your visit. If you are uninsured or prefer to pay out-of-pocket, self-pay options are available — please contact our office directly for details.
Cancellation Policy
We kindly ask that you provide at least
48 hours’ notice if you need to cancel or reschedule your appointment. Late cancellations and no-shows may be subject to a fee. To cancel, please call our office directly or use the link provided in your confirmation email.
Appointment Preparation
Some appointments may require specific preparation, such as fasting, completing forms in advance, or obtaining a referral. If your visit has any preparation requirements, detailed instructions will be included in your confirmation message. Please review and follow these instructions carefully, as failure to do so may result in a delay or the need to reschedule.
