Welcome to Mental Alive

Mental Alive provides psychiatric care for adults age 18 and older, with in-office appointments in Palm Beach Gardens and telehealth appointments throughout Florida. Standalone ADHD testing is available for adolescents and adults. Services are available in English and Creole.

When completing the booking form, please enter a brief reason for your appointment in the “Reason for Visit or Comments” field. Although the form labels this field as optional, this information is important and helps us prepare for your appointment. Please provide only a brief reason and do not include your complete medical history.

Please scroll down and select the service that best matches your needs:

  • New psychiatric patients: Select “New Patient Psychiatric Evaluation” and choose in-office or telehealth.
  • Returning patients: Select “Medication Follow-Up” and choose in-office or telehealth.
  • Standalone ADHD testing: Select “Standalone ADHD Testing: Results Only” or “Standalone ADHD Testing + Brief Clinical Interpretation.” Testing may be completed remotely or in the office.
  • Full adult ADHD evaluation: Select the self-pay ADHD evaluation bundle or the insurance ADHD evaluation, then choose in-office or telehealth.
  • Alcohol or opioid use treatment: Select “Substance Use Treatment Initial Evaluation” and choose in-office or telehealth.
  • Spravato treatment inquiry: Select “Spravato Screening Consultation.”
