Hormone Replacement Therapy - Initial Evaluation
Your initial consultation fee is $295. A $75 deposit is required at the time of booking and will be applied toward your appointment.

If you need to cancel or reschedule, please do so at least 24 hours in advance. Late cancellations or missed appointments will result in forfeiture of the deposit.

During this visit, we will review your symptoms, medical history, and any prior lab work.

If you have recent labs from another provider, please submit them prior to your appointment.

Medical Weight Loss- Initial Consultation 
Your initial consultation fee is $125. A $75 deposit is required at the time of booking and will be applied toward your appointment.
