Please note a $75 deposit is required for all appointments with the exception of lab appointments. Be sure to book an appointment at a time you are available and add it to your calendar.
If you cancel your appointment within 48 hours, your card will be charged this fee as outlined in policy and procedures. If you No-show your appointment you will be billed the full amounts of the appointment, to the card on file. Please refer to Policies and Procedures for more information. 


Initial Consult- All new patients 90 Min $ 225.00

  • In the email there will be links to forms- ALL forms must be completed prior to your appointment.
60 minute visit 60 Min $ 225.00
Choose this visit if you:
  • Are enrolled in weight loss or metabolic programs and are scheduling your monthly 60 minute visit 
  • Need a longer follow up appointment,
  • Would like an in-depth supplement review
  • Need a DUTCH test review
  • Comprehensive lab review

30-minute visit Follow-up 30 Min $ 110.00
  • This visit is for a follow-up on a previously addressed health issues.
  •  If you have a new acute issue, please select 30-minute acute care visit
  • If you have a membership, please select membership visit. 
  • If your visit runs over 30 minutes you will be billed $55 for every 15 minutes over, please choose the appropriate visit length 
Telehealth Follow-up visit 30 Min $ 90.00
  • Choose this option if you desire a telehealth appointment. 
  • appointments may be 15-30 minutes in length. 
  • Telehealth is appropriate for lab and medication/supplement review, Peptide check ins and simple, straight forward visits such as nutritional concerns. 
  • Telehealth is not appropriate for blood pressure checks, weight loss visits, acute injury, cough, ear pain, sore throat, or anything that entails a physical exam. 
Acute care visit 30 Min $ 110.00
  • MUST BE AN ESTABLISHED PATIENT TO BE SEEN FOR ACUTE CARE
  • If you have never been seen at Revitalize Health, please go back and select "New patient consult." 
  • Select this visit if you have a new onset concern such as acute illness, injury or side effects. Must be a current patient. 
  • If you have a membership, please choose membership visit.
