Book this VIRTUAL appointment if you are a new client interested in our Weight Loss program.

This appointment requires a non-refundable/not transferrable deposit of $150 which covers your initial evaluation and review of medical history. The $150 deposit can be applied to any treatment plan or medication.
*Note that if you have been absent from the program for 6 months or more, you will be required to pay a $150 initial consultation fee to begin your program again.

Labs Required: Labs must be completed and resulted prior to your initial consultation so plan to get your labs drawn as soon as possible. A lab order will be submitted once your appointment is booked.

Pre-Whitening Instructions:

To avoid possible gum irritation, DO NOT brush your teeth the day of your whitening appointment. You may rinse with mouth wash.
It is recommended that you eat prior to your appointment. You will not be able to eat or drink anything but water for at least 2 hours after your whitening.
