This service includes a full history, body composition assessment, discussing familial/genetic medical concerns, labs, if needed, will be ordered and/or drawn or reviewed if you are bringing them from elsewhere. This is the initial step for any of the following services.

Weight Loss including determining the causes of weight gain, other related health issues, and lifestyle assessment to determine what lifestyle modifications will be doable and what medications will be doable and get you to your goals.
Men's Health including Testosterone Replacement Therapy (TRT) and adjunct therapies to address fertility, erectile dysfunction, male pattern baldness and other related issues
Women's Bio-Identical Hormone Replacement including other frequently related topics such as intimacy wellness, energy levels, libido, insomnia and more
Thyroid Optimization including full assessment of thyroid status, history, PCOS, brain fog, energy levels, depression, etc.

At this visit we will review goals and start a membership program to cover the expected time for you to reach your goals. All specifics will be reviewed together so we are in agreement.

Common tier 1 medications used for most conditions will be included; more expensive medications and any additional labs needed will be discussed so you can make an informed decision about costs.

We spoke and determined that Evergreen Medical Wellness is a good fit for you for periodic primary or walk-in care type services. This 45 minute visit allows us to delve deeper into your medical history, lifestyle and current medical goals, as well as work together to select a plan that you feel fits your goals best. We can also provide basic walk-in care such as medication refills, sick visits, and assessments, small joint injections and more. Though we do not currently function in the comprehensive role of a Primary Care Provider (PCP), we can certainly fill in and are adept at these services when you need them. This visit is billed in the following time increments so you will know exactly what to expect.

Essentials Plan: Appetite suppressant medication is utilized with monthly follow-up. $109 - $159 depending on medication chosen. This is not a membership based service.
Vitality Membership Program: Semaglutide and all supplies are provided with this plan which includes weekly telemedicine accountability follow-ups. This is our most successful program, and costs $350 per month.
Tirzepatide Membership Program: Tirzepatide is utilized to help with weight loss and diabetes, and is helpful for those who are no longer finding success with other medications. Tirzepatide is provided with all supplies and is dosed per vial. The membership is $149 per month, including weekly follow-ups and the tirzepatide is $499 per vial.
