New Patients

- Select a NEW ASSESSMENT - TELEHEALTH below.
- You will then be able to pick an appointment time that works for you. 
- You will then put in some basic information, including your credit card, in order to reserve your appointment. 
- You will need to complete all intake forms through the patient portal before your appointment.
- If you fail to complete all forms before the appointment you will be charged a Cancelation fee ($100) and have to reschedule for a different appointment. 
 

Existing patients

- Select an FOLLOW UP - TELEHEALTH below. 
- You will then be able to pick an appointment time that works for you. 
- You will then put in some basic information, including your credit card, in order to reserve your appointment. 
- You will need to complete all intake forms through the patient portal before your appointment.
- If you fail to complete all forms before the appointment you will be charged a Cancelation fee ($100) and have to reschedule for a different appointment.
