Please complete your lab draw first, then schedule your initial telehealth consultation for no earlier than 15 days after your lab draw so results are available for review. You may request available appointment times through the patient portal.

Start with clarity, not guesswork. This comprehensive first-step assessment is designed to uncover the root causes of weight gain and chronic health concerns. It includes a detailed health history review, functional interpretation of baseline labs, and a personalized written plan outlining clear next steps for care. This assessment focuses on information gathering and planning and does not include follow-up visits, ongoing messaging, or membership access. Baseline laboratory testing is required and billed separately at cash-pay rates.
Root Revive Prime Membership is a higher-support, structured care membership for patients who need closer follow-up, frequent plan adjustments, and faster progress during an active improvement phase.

Membership includes:
• 1 scheduled visits per month (30 minutes)
• Medication management when clinically appropriate (prescribing, refills, monitoring, dose adjustments)
• Lab/testing management (ordering, interpretation, trend tracking, plan updates)
• Secure portal messaging for non-urgent questions with response within 2 business days (Tues–Thu)
• More frequent care plan adjustments and coordination appropriate during the Prime phase
• 10% off medical-grade supplements when purchased through Essence Integrative Care

Commitment length:
• 6 months
• Once progress is stable and monitoring needs decrease, patients may transition to Root Revive Maintenance Membership by clinician recommendation.

Not included / paid separately by patient:
• Medication costs (billed by the pharmacy/compounding pharmacy)
• Lab/testing fees (billed by the lab; estimated costs reviewed prior to ordering)
• Optional specialty testing (if applicable)

Policies:
• Visits must be scheduled in advance and do not roll over month to month
• Missed/unused visits are forfeited• Messaging is for non-urgent issues only; not for emergencies (urgent symptoms: call 911 or go to the ER)
• New or complex concerns may require a scheduled visit
• Responses outside of Tues–Thu may occur as a courtesy but are not guaranteed.

Billing occurs monthly on the same calendar date you enroll.

Agreement: I understand this is a $399/month membership with a 6-month commitment and includes 1 scheduled 30-minute visit per month, medication and lab/testing management, and non-urgent secure portal messaging with replies within 2 business days (Tues–Thu). Medication and lab/testing fees are paid separately to the pharmacy and lab. Visits do not roll over. Responses outside of Tues–Thu may occur as a courtesy but are not guaranteed.
