Booking Confirmation Instructions

After booking your appointment, please check your email (including your spam/junk folder) for a confirmation message from our office. This email will contain everything you need to prepare for your visit, including:

- Required intake forms and consent documents

- Instructions on any lab work needed prior to your appointment

- Office location, parking, and arrival time details

Please complete all forms and follow the preparation instructions before your appointment so we can make the most of your time together. If you do not receive the email within 24 hours, please contact our office. It is important to complete all listed items prior to the appointment or it will be canceled with no refund. Please complete labs as soon as possible as it can take time for some results to come in.
