During your Complimentary Discovery Call with our Wellness Coordinator/Health Coach for an introductory intake and discovery session. During this call, we will:
Review your medical history, current concerns, symptoms, and health goals
Complete required questionnaires and intake forms
Educate you on how to use the Optimantra patient portal for communication, scheduling, and access to your records
Schedule your lab draw (results typically return in 5-7 days)
Provide an overview of our wellness program, including:

This call ensures your care is personalized and well-organized. Once lab results are available, we will contact you to schedule your Initial Wellness Appointment with one of our Nurse Practitioners to create your customized wellness plan. During your 80-minute appointment, your Nurse Practitioner will dive deep into your concerns, health status, past medical history, surgical history, lab values and create a wellness plan to address your needs and get to the the root cause complaints. Treatment includes but is not limited to weight loss therapies, hormone and thyroid optimization, peptide therapies, etc.
