At your consultation, you will meet with the provider and have the option to have labs done in-house. If you have had labs drawn within the past 3 months, you may e-mail the results ahead of time to [email protected] or bring them with you for the provider to review.

On the next page, please select the best day and time for your appointment. You may fill out intake forms in our patient portal, or arrive 15 minutes early and fill them out upon arrival.
