A $60 deposit is required for new patient visits and sick visits and will be applied to your account at time of visit; deposit will not be refunded for missed appointments. Payment in full is due at time of service. You will be prompted to enter a card at time of appointment scheduling for deposit collection (additional 3% transaction fee for all card payments).

New Client Establish Care - BARIATRIC CONSULTATION/COUNSELING 45 Min

Initial visit for patients wishing to establish care with Prestige Wellness for Bariatric Consultation. This visit is for first visit clients and will last approximately 30-45 min. During this visit, patients will meet with our provider in a relaxed environment for an initial examination, medical and surgical history, and discussion of treatment options and plan of care. Patients should hydrate well for 24 hours prior to the appointment and preferably arrive in a fasting state as lab may be drawn at this appointment if patients elect to enroll in a treatment program. Please complete portal sign up and any associated forms prior to your appointment.

New Client Establish Care - WEIGHT LOSS 60 Min

Initial visit for patients wishing to establish care with Prestige Wellness for medically supervised weight loss. This visit is for first visit clients and will last approximately 60min. During this visit, patients will meet with our provider in a relaxed environment for an initial examination, medical and surgical history, and discussion of treatment options and plan of care. Patients should hydrate well for 24 hours prior to the appointment and preferably arrive in a fasting state as lab will be drawn at this appointment if patients elect to enroll in a treatment program (you may take any prescription meds as scheduled and drink water). New Client Establish Care - WOMEN'S HORMONE REPLACEMENT THERAPY 60 Min

Initial visit for patients wishing to establish care with Prestige Wellness for bioidentical hormone replacement therapy (BHRT). This visit is for first visit clients and will last approximately 60min. During this visit, patients will meet with our provider in a relaxed environment for an initial examination, medical and surgical history, and discussion of treatment options and plan of care. Patients should hydrate well for 24 hours prior to the appointment and preferably arrive in a fasting state as lab will be drawn at this appointment if patients elect to enroll in a treatment program (you may take any prescription meds as scheduled and drink water). New Client Establish Care- GENERAL WELLNESS INJECTIONS/INFUSIONS 60 Min

Initial visit for new clients wishing to establish care for general wellness services such as oral and injectable supplements, infusions, and general wellness consultations. Wellness clients will also have access to our lab services (additional charge) with on-site sample collection and next day results. Your first visit is a bit longer due to need to establish client/provider relationship; follow up for lab results and discussion regarding other supplements and injections may be done via phone/telemed with quick "in-and-out" injection visits included in the price of your supplement.

Lab- TESTOSTERONE FOLLOW UP LAB (9855 +0004CMP) 10 Min

Lab panel following initial treatment with hormone therapy. This is used to assess efficacy of treatment and adjust dosage if needed and is required for ongoing treatment. Please hydrate well 24 hours prior to your visit and arrive in a fasting state if possible other than water (you may take any prescription meds as scheduled). Lab in this panel includes:

-Free testosterone
-Total Testosterone
-SHBG
-Estradiol (E2)
-PSA
-CBC
-CMP
-Lipid Panel

Follow Up TESTOSTERONE (MALE) Appointment - Established Patients Only 15 Min

Follow up visit for male patients on testosterone replacement therapy.

Follow Up WEIGHT LOSS Appointment - Established Patients Only 15 Min

Appointment booking for established patients returning for WEIGHT LOSS follow up appointment.

Follow Up WELLNESS INJECTION Appointment - Established Patients Only 10 Min

Follow up appointment for wellness injection for established patients only. This appointment is for injections only and does not book for infusion therapy due to time. If you are interested in booking an infusion session, please call the practice as infusion therapy is by special order only.

Follow Up BARIATRIC Appointment - Established Patients Only 25 Min

Follow up appointment for bariatric counseling patients.

Office visit for minor illnesses such as the conditions listed below:

respiratory infection
sinus infection
Covid/Flu
ear infection
Strep
UTI
yeast infection
rash/allergic dermatitis
pink eye
headache/migraine

Testing is available for covid/flu, strep, UTI, mono, pregnancy for an additional $15 charge

Anti-inflammatory, antihistamine, antibiotic, ketorolac injections $30 each

Bloodwork is additional and based on selected panel

Your $60 deposit will be applied to the visit total and any additional charges will be due at the completion of your visit. If you are ordered for any prescription medication, we will happily send it electronically to your pharmacy for pick up.
