NEW PATIENTS - All new patients start with a consultation visit
- This visit may last up to 55 minutes
- The consultation can be done in person or through a telehealth session - During a consultation visit, a thorough history will be obtained, and a treatment plan will be recommended
RETURNING PATIENTS - Returning patients can be seen in person or via telehealth for follow-up
- This visit may last up to 25 minutes - Please choose your preferred appointment type at the time of booking.
TELEHEALTH APPOINTMENTS - For patients seeing Ann Konkoly CNP, you must be in the State of Ohio, New York or Arizona at the time of your visit to receive care - For patients seeing Holly DeWalt CNP, you must be in the State of Ohio at the time of your visit to receive care
- For patients seeing Jean Marino CNP, you must be in the State of Ohio at the time of your visit to receive care - For patients seeing Shaylinn Romano CNP, you must be in the State of Ohio at the time of your visit to receive care
IN PERSON APPOINTMENTS - In person visits are available at 24300 Chagrin Blvd, Suite 103, Beachwood, Ohio 44122 - The office is located inside Shaker Women's Wellness in the Chagrin Commerce Building on Chagrin Blvd in Beachwood, Ohio
- We are on the first floor in Suite 103.
- Please check-in at the front desk when you enter.
