A valid credit card is required to book. No-shows or cancellations under 24 hours incur a $50 fee.
 

Wellness Culture operates on a direct-pay model. Payment is due at the time of service.

This appointment is an initial phone consultation for new patients to our Testosterone optimization. Even if you have been seen by Wellness Culture for other services, you would be considered a new testosterone patient. Please read fully our procedure process. 
  1. 15-30 Minute Phone Consultation (Required First Step) - We will begin with a brief phone consultation to review your health history, current symptoms, and wellness goals, and to determine whether hormone therapy is appropriate for you.
  2. Comprehensive Lab Panel - Following your phone consultation, we will arrange comprehensive laboratory testing. Labs must be completed before your in-office visit and include a full hormone and metabolic panel to ensure treatment is safe, effective, and tailored to your needs. Lab services must be prepaid before lab orders are submitted, after which you may schedule your lab appointment.
  3. In-Office Visit + Personalized Care Plan - Once your lab results are available, you will be contacted to schedule an in-office visit. During this appointment, we will review your labs, discuss treatment options, and develop a personalized hormone optimization plan. If appropriate, treatment may begin at this time. Your treatment subscription begins once you choose to proceed with the recommended plan.
This program is not covered by insurance and is offered on a self-pay basis. All lab services are prepaid at the time of scheduling.
This appointment is for new male hormone replacement therapy patients who have completed their phone consultation and laboratory testing and are ready to begin care at Wellness Culture. During this in-clinic visit, we will review your health history, training and lifestyle factors, and go through your laboratory results together to determine how your hormones are currently functioning.

Your labs will be explained in detail so you understand what is optimal vs. normal and how testosterone, metabolic markers, and overall physiology relate to energy, strength, recovery, libido, mood, and long-term health. Based on this evaluation, your personalized protocol will be initiated and medication options, dosing strategy, administration technique, and supportive therapies will be discussed.

Please have all required forms and questionnaires completed prior to arrival to allow adequate time for education and treatment planning.

This In office visit is for established patients enrolled in testosterone therapy at Wellness Culture. Please complete your follow-up questionnaire prior to arrival.
During your visit we will review your progress, symptoms, and labs (when applicable) to evaluate how your treatment is performing. Adjustments may be made to dosing, injection frequency, delivery method, and supportive medications to keep therapy safe and optimized.

This program is self-pay. Payment is collected at the time of service as part of your ongoing care plan.
This virtual visit is for established patients enrolled in testosterone therapy at Wellness Culture. Please complete your follow-up questionnaire prior to your appointment. We will review your progress, symptoms, and labs (when applicable) and make any necessary adjustments to dosing, medications, and supportive therapies to maintain safe and effective treatment.
