• Services are available to patients physically located in Alabama at the time of their telehealth appointment.
• Revive is a self-pay practice and does not bill insurance.
• Initial consultations are required for all new patients before treatment recommendations can be provided.
• Laboratory testing and medications are not included in visit fees and may result in additional costs.
• Patients seeking ongoing care may choose between the Revive Monthly Membership or pay-as-you-go follow-up visits.
• Telehealth visits require a reliable internet connection and a device with audio and video capabilities.
• Appointments canceled with less than 24 hours notice or missed without notice may be subject to a cancellation or no-show fee in accordance with clinic policies.
• Revive does not provide emergency medical care. If you are experiencing a medical emergency, call 911 or seek immediate medical attention.
