• Services are available to patients physically located in Alabama at the time of their telehealth appointment.
• Revive is a self-pay practice and does not bill insurance.
• Initial consultations are required for all new patients before treatment recommendations can be provided.
• Laboratory testing and medications are not included in visit fees and may result in additional costs.
• Patients seeking ongoing care may choose between the Revive Monthly Membership or pay-as-you-go follow-up visits.
• Telehealth visits require a reliable internet connection and a device with audio and video capabilities.
• Appointments canceled with less than 24 hours notice or missed without notice may be subject to a cancellation or no-show fee in accordance with clinic policies.
• Revive does not provide emergency medical care. If you are experiencing a medical emergency, call 911 or seek immediate medical attention.

Medical recommendations are only provided during a scheduled Initial Consultation.

There is no cost and no obligation to continue. This is simply an opportunity to learn more and determine whether Revive is the right fit for your health journey.

Initial Consultation 40 Min

This comprehensive new patient consultation is designed to evaluate your overall health, weight history, lifestyle habits, medical conditions, current medications, and weight loss goals. During this visit, we will discuss factors that may be impacting your metabolism, review available laboratory data, and develop a personalized treatment plan tailored to your needs. Treatment recommendations may include nutrition and lifestyle modifications, behavior change strategies, laboratory testing, and prescription medication when clinically appropriate.

This visit is intended to establish care and create a foundation for long-term success. Medication costs and laboratory fees are separate and are not included in the consultation fee.

Membership Follow Up 20 Min

The Revive Monthly Membership provides ongoing medical weight management support and accountability. Membership includes one follow-up visit each month to monitor progress, review treatment response, adjust recommendations as needed, and address questions related to your care plan.

Members receive continued guidance focused on sustainable lifestyle change, metabolic health improvement, and long-term weight management. Treatment recommendations may include nutrition counseling, physical activity guidance, laboratory monitoring, and medication management when clinically appropriate.

Membership fees cover clinical support and follow-up care. Laboratory testing and medications are billed separately.

Non-Membership Follow Up 20 Min

This follow-up appointment is available for established patients who are not enrolled in the Revive Monthly Membership program. Patients must have completed a follow-up appointment with Revive within the previous 90 days to remain eligible for this visit type. Patients who have not been seen within the last 90 days may require a more comprehensive reassessment before treatment can continue.

During this visit, we will review your progress, discuss any challenges or concerns, evaluate treatment effectiveness, and make adjustments to your care plan as appropriate. This visit may include medication management, laboratory review, lifestyle counseling, and recommendations to support your ongoing health and weight loss goals.

Laboratory testing and medication costs are separate and are not included in the visit fee.

Re-Establishment Visit 20 Min

This appointment is intended for established patients who have not completed a follow-up visit with Revive Total Health & Wellness Clinic within the previous 90 days or who require a comprehensive reassessment of their health status and treatment plan.
During this visit, we will review your medical history, current health concerns, medications, laboratory results, progress since your last appointment, and any changes that may affect your treatment recommendations. A comprehensive reassessment allows us to ensure that your care plan remains safe, appropriate, and aligned with your current goals.

Treatment recommendations may include medication management, laboratory testing, lifestyle counseling, and modifications to your personalized treatment plan. New prescriptions, dose adjustments, or continuation of therapy may require updated clinical information and laboratory evaluation when clinically appropriate.

Laboratory testing and medication costs are separate and are not included in the visit fee.
