TLC Healthcare Solutions – Cancellation Policy

At TLC Healthcare Solutions, we are committed to providing exceptional care and service to all of our clients. In order to maintain the highest standards of care and ensure availability for all who need our services, we kindly ask that you observe the following cancellation policy:

Appointment Cancellations

  • We require at least 24 hours' notice for any appointment cancellations or rescheduling.

  • Cancellations made less than 24 hours in advance may be subject to a cancellation fee of  $50.00 dollars

  • No-shows without prior notice may result in a full appointment fee being charged.

How to Cancel

  • You may cancel or reschedule your appointment by calling us at 959-301-9606 or emailing us at [email protected].

  • Please include your name, appointment date/time, and reason for cancellation.

Exceptions

We understand that emergencies and unforeseen circumstances may arise. Exceptions to this policy will be considered on a case-by-case basis.

Repeated Cancellations

Clients with a pattern of missed or late cancellations may be required to prepay for future appointments or may be placed on a standby list for future bookings.

New Patient Consult, which includes History & Physical, review of medications, other specialist providers, and to discuss health problems/concerns
Health concern or illness that needs attention or treatment
Follow up appointment to discuss health concerns, medications, plan of care, lab or testing results
For generally healthy individuals who want better access and preventive care.

Includes: 
  • 1 annual comprehensive physical exam
  • 6  virtual or in-person visits/year (office or home)
  • Unlimited secure messaging with provider
  • Same-day/next-day appointment scheduling
  • Annual lab review & basic wellness screening
For patients with mild to moderate chronic conditions (e.g., hypertension, hyperlipidemia). ?

Includes:
  • Everything in Essential Tier
  • 6 virtual or in-person visits/year
  • Quarterly chronic disease management reviews
  • Basic in-home or office diagnostics (e.g., BP, glucose)
  • Labs would be additional cost
For patients with multiple chronic conditions or complex care needs.

Includes: 
  • Unlimited visits (in-person or virtual)
  • Medication management
  • Care coordination with specialists
  • Home visits 2x/year (optional)
  • Annual executive-level physical
  • 24/7 access by phone/text
Annual Membership Fee $1,200/year (equivalent to $100/month, billed annually). 

Includes:    
  • 1 comprehensive annual wellness exam
  • 6 visits per year, Priority same-day/next-day scheduling
  • Access to direct messaging and telehealth
  • Basic lab work (1x/year)
  • Coordination of care/referrals
