This comprehensive service package is designed to assist patients with the completion of essential paperwork required for work, school, or other personal needs. The package covers a range of documentation, including but not limited to Family and Medical Leave Act (FMLA) forms, disability paperwork, return-to-work/school notes, and other third-party forms requiring provider input.

Fee Structure:

• Flat Rate: $100 for up to 60 minutes 
• Payment Policy: This service is not billed to insurance, as paperwork completion is not considered a billable treatment or clinical service. Payment is due at the time of service.
