This appointment is for your general wellness panel -

We will draw your labs - fasting with 2 large glasses of water prior to arrival.

We will assess your kidney function, liver function, cholesterol levels, vitamin D-B12-Magnesium, Insulin, A1C, Thyroid and complete blood counts. 

You will come into our office get your labs drawn and Carly will see you via Telehealth in 1 week to go over lab results and give your recommendations on lifestyle, supplements or referrals as needed.

Here’s how it works:
Schedule Your Initial HRT Consultation with Carly
We’ll meet virtually to discuss your symptoms, health history, and the basics of hormone therapy (HRT).
Get Labs — done locally at your nearest clinic/hospital/etc
We’ll send an order to your preferred clinic or hospital lab.
Once collected, results are faxed directly to Revive Wellness & Weight Loss.
Follow-Up Visit (1 Week Later)
We’ll go over your lab results, discuss your treatment plan, and get your hormones on track — together.

Telehealth HRT Pricing
Initial Consultation: $250 Includes full assessment, lab order, and your first month of testosterone supplies if you decide to start therapy.
Ongoing care billed monthly at $125 (includes check-ins, symptom support, and med management)

Carly will email consents - we will discuss your medical history, give you a tracking application, set goals for your intake and activity as well as medication management. Approximately 30-minute appointment. Patient will need their weight/height and other pertinent medical information.
