Please be sure to select the correct Appointment Type related to the appointment and services you are requesting. 

For example, if you are a new patient that prefer a telehealth appointment, please select Telehealth New Patient appointment type. 
If you are a follow up patient and prefer an in-office appointment, please select an In Office Follow Up appointment type. 
If you are only interested in Botox, please select a Neurotoxin appointment type. 

Let us know at least 24 hours prior to your scheduled appointment if you need to reschedule or cancel, you can do that through your patient portal or call/text us at 941-882-0599.
