Deposit/Consultation fee is non-refundable. At provider's discretion - this fee may be applied to the first month of a program if you choose to enroll within 14 days of the consultation. Does not apply to primary care memberships. By scheduling your appointment and paying the deposit, you are agreeing to these terms.
Cancellation Policy: If you cannot keep the scheduled appointment, you MUST notify Amanda Conroy to cancel or reschedule the appointment 24 hours before the scheduled appointment time. There is a cancellation fee of $150.00 for a scheduled 45-60-minute session when an appointment is missed and/or cancelled within the 24-hour period. If you are late for a session by 15 min or more that session will be cancelled, and you will be charged a cancellation fee of $150.00 for a scheduled 45-60-minute session. If you cancel or reschedule multiple times, we may re-evaluate your needs, desires, and motivations for treatment at this time.
