This is an in-person, meet-and-greet type of service which includes having your vital signs taken and a 20-minute consultation with our provider to discuss healthcare needs and if we can provide you with the care you need.
This is the first appointment to establish care. This includes a 45 minute visit with our provider to discuss medical history, review medications, and discuss future goals/needs.
This is a medical appointment designed for rapid assessment and treatment of sudden, short-term health issues. Common reasons for this visit are infections, respiratory issues, minor injuries, skin/allergies, or digestive issues.
Monthly visits for chronic care management. This visit is a continuous partnership with your provider to help stabilize your health by offering monthly vital sign check-ups, lifestyle coaching and goal setting, care plan reviews, we provide education with support, as well as keep the coordination of care between family members and/or multiple specialists organized.
Wellness visits focused on proactive health management through routine screenings, lifestyle education and counseling with support, and risk assessment to prevent disease. This can include regular check-ups, blood pressure screenings, cancer screenings with referrals as needed, and screenings for conditions such as diabetes, anxiety, or depression.
A virtual visit to discuss and treat sudden, non-emergent health concerns. This can include respiratory issues such as allergies, mild coughs, and sinus congestion, skin issues such as new rashes, bug bites, minor burns, or eczema flare-ups, digestive issues such as nausea, vomiting, or minor stomach upsets, as well as miscellaneous conditions such as pink eye, uncomplicated urinary tract infections, and headaches.
This is a virtual visit for established patients to monitor chronic conditions with the use of remote patient monitoring, lifestyle coaching and goal setting, care plan reviews, provide education and support, as well as coordination of care between family members and/or multiple specialists.
This is similar to the Maintenance Phase and includes one initial consult and one in-person or telehealth appointment per month to discuss progress or any negative effects.

This is an initial appointment to discuss goals, go over plans of action, and establish a relationship. This consult lasts up to 1 hour and the fee will go towards your Foundation Phase Cost.
MUST HAVE WEIGHT LOSS INITIAL APPOINTMENT FIRST. This appointment is a follow-up to an initial appointment. These appointments are shorter in time compared to an initial visit. This includes having vital signs taken and discuss reactions to the medication.
Scheduled over-the-phone appointment to follow-up after an initial or acute appointment. If labs or other services are required, appointment will change to an acute appointment.
