This can include regular check-ups, blood pressure screenings, cancer screenings with referrals as needed, and screenings for conditions such as diabetes, anxiety, or depression.
This is a medical appointment designed for rapid assessment and treatment of sudden, short-term health issues. Common reasons for this visit are infections, respiratory issues, minor injuries, skin/allergies, or digestive issues.
Monthly visits for chronic care management. This visit is a continuous partnership with your provider to help stabilize your health by offering monthly vital sign check-ups, lifestyle coaching and goal setting, care plan reviews, we provide education with support, as well as keep the coordination of care between family members and/or multiple specialists organized.
A virtual visit to discuss and treat sudden, non-emergent health concerns. This can include respiratory issues such as allergies, mild coughs, and sinus congestion, skin issues such as new rashes, bug bites, minor burns, or eczema flare-ups, digestive issues such as nausea, vomiting, or minor stomach upsets, as well as miscellaneous conditions such as pink eye, uncomplicated urinary tract infections, and headaches.
This is a virtual visit for established patients to monitor chronic conditions with the use of remote patient monitoring, lifestyle coaching and goal setting, care plan reviews, provide education and support, as well as coordination of care between family members and/or multiple specialists.
