Please review the following prior to booking your visit.

*You must be 18 years of age to treatment.

*You must present state ID or Driver License at the time of visit.

*You must also be present in your primary state of residence during visit. For example, if you are Texas resident you need to present in the state of Texas at the time of visit to remain compliant with the law. 

*If you don’t have recent lab work(lab work greater than 90 days), labs can be ordered for you.  You can email completed labwork to [email protected]

However, I will not prescribe your medications without completed lab work. 

*If you need to cancel, don’t fret. Just be sure to contact me directly at 469-392-5156. 

*If you are a no call/no show,  you will be charged a $25 no show fee prior to booking future appointments.
