You may schedule your initial appointment online at a time that works best for you. If you need assistance with scheduling, please feel free to contact us—we are happy to help. Please note that all appointments require provider confirmation. Once your appointment is confirmed, you will receive an email invitation to create your Patient Portal account. After logging in, please complete all required intake forms at least 24 hours before your appointment to avoid cancellation.

After accessing your Patient Portal, please also upload a copy of your driver’s license and the front and back of your insurance card. This can be done in the “Documents & Handouts” section using the left navigation bar. Select the appropriate document type and upload your files.
