NEW PATIENT - Discovery Wellness Consultation 60 Min
Sick and tired of feeling sick and tired? Anxious? Sleeping issues? Low libido? Exhaustion? We have solutions and personalized treatment options available! 

During this initial consultation, we will review your wellness forms and go over your concerns to gather the necessary information and discuss the plan for a lab draw. 

About 10 - 14 days after your labs are drawn, we will meet again to review your labs and develop a personalized treatment plan tailored to your needs which may include our customized treatment options which may include hormone replacement therapy, medically managed weight loss, functional medicine options and more.

Fee: $200
* The Initial Consultation is a long appointment and in order to set aside this amount of time for a single patient, IT MUST BE PRE-PAID.

After booking this appointment, you will receive an email with a throughout medical and history intake form, consent forms and information on how to register for our Patient Portal. Lab Review and Treatment Plan Appointment 30 Min
Post-Discovery Wellness Lab Review & Treatment Plan:

During this visit, we will review your lab results, explain key findings, and create a personalized treatment plan. Based on your health goals, recommendations may include lifestyle changes, supplements, hormone therapy, medical weight loss or other wellness treatments. 
Hormone Replacement Therapy - FOLLOW-UP Visit 20 Min

This follow-up appointment is for established Hormone Replacement Therapy patients. 


During this appointment we will review lab results as needed, discuss any new or existing symptoms, renew medications when applicable and adjust your treatment plan, if needed. 

Medically Managed Weight Loss - FOLLOW-UP Visit 20 Min

This follow-up appointment is for patients who are participating in Medically Managed Weight Loss with our clinic. 

 
During this appointment we will review lab results as needed, discuss any new or existing symptoms, renew medications when applicable and adjust your treatment plan, if needed.
Functional Medicine - FOLLOW-UP Visit 20 Min

This follow-up appointment is for established Hormone Replacement Therapy patients. 

 
During this appointment we will review lab results as needed, discuss any new or existing symptoms, renew medications when applicable and adjust your treatment plan, if needed. 
New Patient: Chronic Care Management Visit 30 Min
Urgent Care Telehealth Visit 20 Min

Schedule this appointment if you would like to be seen as soon as possible for an illness that needs immediate treatment like strep throat, flu/cold, urinary tract infection, an ear infection or any other acute illness.
