This will allow us to begin the registration process, create your patient chart and confirm your appointment/lab orders for your next visit.

New patients: select “New Patient Intake Call.”

TMED - Intake Call 30 Min $ 100.00

Initial appointment via telemedicine to review your intake of concerns/goals, and ensure we are a good fit for your needs. You will receive intake documents to complete prior to your visit. Any recent labs (< 6months) may be submitted for review during your appointment.

EXISTING PATIENT - Registration/Appointment Request 45 Min $ 0.00

Existing patients who wish to request transfer/registration for appointments scheduled after 4/1/2026.
Please note, if you are an existing patient being seen prior to April 1, 2026, your appointment will continue via previous clinic platform.
