Please Read Before Booking
 
Please carefully read each appointment description to ensure you select the correct visit type.
 
- All memberships require a New Patient Consultation before enrollment.
- IV infusions and injections require medical screening if you are not already an established patient.
- If you are unsure which appointment type to select, please call or text 859-712-0955 and we will help guide you.
- We encourage first visits to be in person, but telehealth may be appropriate in some cases. If you are unsure, feel free to message us.
 
Payment & Insurance
 
We do not bill insurance for office visits or in-office services. This allows us to provide transparent pricing so you know your cost up front and will never receive a surprise bill from our office.
 
- A $100 non-refundable deposit is required to book your office visit and will be applied to your visit cost.
- For injections or IV therapy, non-redundable deposit equal to 50% of the service cost is required at the time of booking. 
- One courtesy reschedule is allowed to transfer the deposit to a new appointment.
- If the appointment is canceled without rescheduling, the deposit will be forfeited.
- Deposit links are sent through Valor and must be paid within 24 hours or the appointment will be canceled.
- Full payment is due at the time of service. We accept cash, debit, credit, and HSA/FSA cards. A 3% processing fee applies to credit cards only. Financing is available through Cherry.
 
Using Insurance
 
- You are welcome to use your insurance for labs and medications through the lab and pharmacy of your choice. These providers will bill your insurance directly.
- For those without insurance or who prefer not to use it, we offer affordable self-pay lab options and pharmacy discounts.

New Patient Consult $ 300.00
The Initial Consultation is a comprehensive 60 minute session designed to understand your health and wellness needs. This service allows me to gather important background information, discuss your goals, and develop a personalized plan tailored to your needs. Whether you’re seeking support for specific health concerns or looking to optimize your overall well-being, this appointment sets the foundation for your journey.


Former Patient-First Visit $ 225.00
First Visit (New to Prevent & Sustain Medicine)
Even if you’ve worked with Chastity before in a provider-patient relationship, your first visit at Prevent & Sustain Medicine is a fresh start! -Choose this option if this is your first appointment at Prevent & Sustain Medicine-


This 60 minute comprehensive visit allows us to review your history, assess your current health status, labs, and current treatment plan.  We'll assess what's working, address any changes, and either continue your existing approach or fine-tune your plan to better support your goals. 



Follow-up-Established Patients Only $ 150.00
For existing patients only.  Includes lab review, treatment adjustments, symptom monitoring, and continued management of your care plan.

Establish Patient Acute/Sick Visit $ 100.00
  • This is a focused, problem-based visit for established patients who develop new symptoms, feel unwell, or need guidance for a short-term concern between routine appointments.
  • These shorter visits are intended for acute issues such as minor illness, infections, medication questions, or new symptoms.  This appointment is not intended to replace scheduled follow-up visits for chronic condition management, ongoing medication management, or comprehensive care planning.  
  • Your visit fee includes medical evaluation and in-office testing as clinically indicated.
  • Telehealth may be available when appropriate; however, concerns requiring physical examination, diagnostic testing, or procedures must be evaluated in person.  Final determination of visit format is at the provider’s discretion.
  • IV therapy, medications, injections, and additional treatments are not included in the base visit fee and are billed separately.  If. IV therapy is desired, it must be selected as an add-on at the time of booking, as availability cannot be guaranteed otherwise.
  • Prevent & Sustain Medicine does not operate as a walk-in urgent care facility.
IV Therapy $ 0.00
Select the Injection service you wish to receive below and indicate if you will require a medical screening or renewal as noted below.

*All IV therapy options include 1000 mL or 1 liter of normal saline infusion*
  • If you are not an established patient of Prevent & Sustain Medicine, you are required to have a medical screening and does not replace a comprehensive medical visit.
    • Approval is valid for 6 months.  After expiration, a renewal screening is required prior to receiving additional IV therapy.
    • Initial Medical Screening: $30
    • Renewal Medical Screening: $15

Lab Draw $ 25.00
  • This appointment is for blood specimen collection ordered through Prevent & Sustain Medicine for Labcorp or Rupa Health testing.
  • This visit includes specimen collection only and does not include a medical evaluation or consultation.
  • Labcorp testing fees are billed separately by the laboratory and are not included in the lab draw fee.  
  • Rupa Health testing fees will be paid at time of lab draw if not paid prior.
  • Please follow all preparation instructions provided with your lab order.  Some tests require fasting or must be completed at specific times of day (such as morning collections).  It is your responsibility to schedule this appointment accordingly. 
Bladder Cocktail Treatment $ 150.00
  • This appointment is for established patients experiencing a known bladder flare who require a bladder instillation treatment.
  • This visit includes urinalysis screening and bladder cocktail administration.
  • This is a procedure-only appointment and does not include a medical evaluation or discussion with the provider regarding new symptoms, diagnosis, or treatment planning.
  • If you are experiencing new or worsening symptoms, or would like to discuss your condition or treatment planning, please schedule an acute visit or follow-up appointment instead.
  • Additional testing or provider evaluation may require a separate visit and/or additional fee.
Injectable $ 0.00
