Appointment will be conducted via HIPPA compliant video consult. Approx 10 minutes prior to your appointment, you will be emailed a link to join the appointment. Appointment is 60 minutes long
300.00 consult fee will need to be paid at the time of booking, non refundable.
After you book online, you will receive an email with all your forms that will need to be filled out prior to your appointment for my review. If they are not filled out we will need to reschedule. Please check your spam folder as sometimes they will get sent into your spam or junk folder.
