This evaluation may include:

• Comprehensive medical history review
• Medication review
• Physical examination
• Review of chronic medical conditions
• Laboratory testing when indicated
• Completion of required surgical paperwork

Important Information

This service is a perioperative risk assessment and is not a guarantee of surgical clearance.
Surgical clearance decisions ultimately rest with the surgeon, anesthesiologist, and procedural facility.
Required laboratory testing may be included when medically appropriate.
An electrocardiogram (EKG) is required for most surgical evaluations.
EKGs are performed through an outside provider or facility.
EKG fees are not included in the perioperative risk assessment fee and are the responsibility of the patient.
Patients are responsible for scheduling and completing their EKG prior to submission of surgical paperwork.
Depending on the patient’s medical history, age, surgical procedure, medications, or underlying medical conditions, additional testing, specialist consultations, or medical records may be required before the assessment can be completed.
Examples may include additional laboratory testing, cardiac clearance, pulmonary clearance, imaging studies, or specialist evaluations.
Any additional testing or consultations required beyond the standard perioperative assessment may result in additional fees, which will be discussed with the patient before services are performed.
Completion of paperwork is contingent upon receipt and review of all required testing, records, and specialist recommendations.
Processing times may vary depending on the timeliness of outside records, EKG results, specialist reports, and additional testing requirements.
