To book your telehealth appointment, please choose the state in which you will be located at the time of your visit, then select the most appropriate appointment type.
Cancellation policy: If you need to reschedule, please do so at least 24 hours in advance. Any no-shows and last minute cancellations with <24 hours notice will incur a $50 fee. A payment method is required on file for all appointment bookings. Please contact me directly for emergency cancellations or concerns.
