For patients experiencing symptoms of menstrual cycle pain, changes in flow or duration, vaginal pain/dryness, painful intercourse, Vaginal discharge, or urination issues please make an IN PERSON appointment.

The first appointment you will book is your Initial Weight Management appointment, either in-office or telehealth, per your preference. This appointment is with Dr. Faleide. Then go in and book your Initial Weight Management - Nutrition appointment, either in-office or telehealth, per your preference. This will be your appointment with our Registered Dietitian, Elise Smith.
