Cash-Pay Practice
Oaks Health Associates is a cash-pay practice. Payment is due at the time of booking unless otherwise noted. We do not bill insurance for our weight management or wellness services.
Forms Required
Please complete all required intake forms, consent forms, and questionnaires before your appointment to avoid delays or rescheduling.
