Please Note Before Booking:
We do not accept insurance.
We are licensed to see patients in Florida.
We provide services for adults ages 18 and up.
Visits are by appointment only.
A $25 deposit is required to secure your appointment.
Deposits are not required for Direct Primary Care follow-up visits.
Your deposit will be deducted from the total cost of your visit.
You may reschedule or cancel your appointment up to 24 hours in advance without forfeiting your deposit.
Cancellations inside the 24-hour window or absolute no-shows forfeit the deposit to cover the cost of the unused clinical time.
Please complete your intake forms at least 24 hours before your visit.

An evaluation for individuals unable to attend work or school due to non-emergent illness or minor injury.

Evaluation to determine whether it is safe to participate in a specific sport.

Examination to ensure a candidate is physically and mentally capable of safely performing specific job duties.
