A $50 booking fee is required to confirm all appointments. This fee is applied toward your visit and is non-refundable in the event of a no-show or late cancellation.
 

For psychiatric appointments using insurance, complete insurance information must be provided at the time of booking. Self-pay services do not require insurance.
 

All patients must upload a valid photo ID prior to their visit. Required intake paperwork must be completed by 3:00 PM the business day before your appointment. Appointments may be canceled if these requirements are not met.
