New Patient Initial Visit

Welcome! Our goal is to identify and address the underlying causes of your symptoms to help you achieve lasting results.
 

During your initial 60-minute visit, your provider will:
- Review your health history, symptoms, and concerns
- Discuss previous and current medications, diagnoses, and lifestyle factors
- Perform a focused physical examination
- Determine appropriate testing, if needed
- Develop a personalized treatment plan based on your individual needs and goals
 

New Patient Visits cannot be scheduled online. To schedule your initial appointment, please call our office at 636-389-2488, and our team will be happy to assist you. Please note: Michelle Wallweber, MSN, FNP-C is no longer accepting male patients.
 

A $50 non-refundable deposit is required to reserve your appointment and will be applied toward the cost of your visit.
 

All intake and consent forms must be completed at least 48 business hours before your scheduled appointment. Appointments may be canceled if required forms are not completed by the deadline. If an appointment is canceled due to incomplete paperwork, it may be rescheduled one time only.
 

IV Therapy & Injection Appointments

If you are scheduling an IV Therapy or Injection appointment, please complete only the forms required for your selected service:
- IV Therapy Consent or the appropriate Injection Consent
- Practice Policies Consent

The New Patient Intake Form is not required for IV Therapy or Injection appointments unless our office specifically instructs you to complete them.
