This is a meet and greet appt only
These visits are for our members and include primary care, weight loss and hormone replacement visit. They are included in your membership.
WE NO LONGER TAKE INSURANCE. If you do not have a membership with our practice you can still be seen as a self pay option. This will allow you to see a healthcare provider, get care when you need it and up to a 30 day supply of medication when appropriate. We do not provide 90 day prescriptions at these visits. You must be a member and receive ongoing care with our practice to receive extended prescriptions.
These appointments are for physicals for employment, sports participation or commerical drivers.
These services are for Intramuscular or Intravenous infusions or vitamins and minerals for to help with overall wellness.
