We’re glad you’re here. Please complete your enrollment paperwork before scheduling an appointment. You can find the forms at tubmanhealth.org/freedom-clinic. Once your paperwork is complete, you can use this page to schedule your visit.

Friendly Reminder: After you choose your appointment type, please select “Already a Client.” The form that appears next is part of the system (we can’t change or hide it), but choosing “Already a Client” helps us make sure your appointment is linked correctly.
