Welcome to NuLife’s online booking platform! After scheduling your appointment, please check your email for required patient forms to complete prior to your visit. If you prefer to fill out your forms in the office, we kindly ask that you arrive 15 minutes early to allow time for completion.

A $25 booking fee is required at the time of scheduling all new patient consults. This fee is non-refundable and will be applied toward the cost of your consult. It also serves as a cancellation/no-show fee if an appointment is missed or canceled within 24 hours. A card on file is required for all patients.”
