Booking Your Appointment:
- Complete the booking form with your accurate personal information.
Insurance Details:
- If you plan to use insurance, please provide your current insurance information accurately.
- Note: Insurance verification may take up to 48 hours. You will be notified once verification is complete.
Payment Information:
- A valid credit card is required to be kept on file for billing purposes.
- Your card will only be charged for co-pays, deductibles, or if your insurance does not cover the service.
Preparing for Your Appointment:
- Arrive at least 10 minutes early to complete any necessary paperwork.
