All intake paperwork must be completed at least 48 business hours prior to your scheduled appointment time. If paperwork is not completed by that deadline, the appointment will be canceled and the card on file will be charged the full intake fee.
Hormone Replacement Therapy is self pay only. The cost for a new patient appointment is $250.00 for 45 minutes. The cost for a follow up appointment is $150.00 for 20 minutes. Your card will not be charged until the time of your appointment.
This appointment is for a psychotherapist consultation with a PMHNP regarding a shared patient's care. It is not a consultation for individual psychiatric services. If you are a patient seeking care, please select a New Patient Appointment instead.
