For current or past 1988 members who have been seen within the last year and want to continue with another 6 months of ongoing, comprehensive care and support.

Graduated Follow-Up Visit
For established patients who have completed their initial membership, are stable on their current plan, and only need periodic follow-up and medication management, typically 1–2 visits per year.

360° Midlife Health Evaluation

All new patient visits are available virtually or in person, depending on your location and preference.

The 360° Midlife Health Evaluation is the first medical appointment for all new patients. After your evaluation and Health Strategy Visit are complete, you may choose to continue with ongoing care through a 6-month 1988 Membership.
